Interesting short video on channel9 on the topic. It is an interview with the Windows usability team, but the disciplines and processes involved are similar across products. Gives a little insight into how we figure out what works and what doesn't for real users. Nothing like sitting on the observation side of a lab to humble anyone who thinks their UI, API design, tutorial instructions, etc are flawless :)
